A large lump is most likely to be a cyst (a tough fluid-filled 'bag'), though a fibroadenoma (a rubbery growth of fibrous and glandular tissue) is fairly common. This sort of lumpiness may result from oestrogen - indicating an oestrogen-dominant hormone imbalance - or from sensitivity of milk gland cells to oestrogen, progesterone or prolactin or to changing levels of these and other sex hormones.
Tight bras can make the breasts congested by obstructing the flow in blood and lymph vessels. The resulting pressure of surplus tissue fluid around the cells might encourage breast tissue to defend itself by forming lumps.
